Palliative & End of Life Care
Receiving news about a life limiting diagnosis is an earth-shattering event. Our goal at Enable Health is to support your emotional, physical and spiritual needs as you transition to new stages of life. Enable Health offers Palliative and End of Life Care services to meet your needs as constant change occurs in your body and your abilities. Your occupational therapist will work with you to help assess and identify areas where you may be having difficulties, and what your goals are. Your occupational therapist will work alongside you as you adapt to changes in your body and abilities.
What to Expect
Your occupational therapist will meet with you in your home environment. Your family members are also welcome to be part of your occupational therapy sessions if you would prefer.
Your occupational therapist will perform an initial assessment with you where we will discuss your medical history, look at how you are managing the activities that you want or need to do on a day to day basis, assess your home and discuss your goals for occupational therapy.
In following sessions, your occupational therapist will work together with you on a specific plan designed to help you meet the goals that were set during the initial assessment. Goals may include being able to help with cooking, to get in and out of your bathtub, or to write letters for your grandchildren.
Through education, modifying tasks or the environment, some occupational therapy activities may include:
Strategies on how to engage in your meaningful activities when they become difficult.
Managing symptoms such as fatigue and breathlessness.
Returning to activities that you previously enjoyed through adaptation or grading the activities.
Completing advanced care planning documentation.
Supporting caregivers to reduce risk of injury, burnout, or other negative experiences.
